Palliative and End of Life Connector Model

This report sets out a practical roadmap for establishing a Tasmanian Palliative and End of Life Care Connector Service.

It responds to a clear problem: when someone is facing a life-limiting illness, the system can feel fragmented, confusing, and hard to navigate. People and families often do not know where to turn, what support is available, or how to connect the pieces of care around them.

The recommendations in this resource are grounded in lived experience, sector consultation and evidence from Tasmania and other states. They outline how a statewide connector service could help Tasmanians find the right information, make meaningful connections to support, and navigate palliative care, end of life care and bereavement with less stress and better outcomes.

This resource is intended for policymakers, funders, service leaders and sector partners who want to strengthen how palliative and end of life care is coordinated across Tasmania.
