Many people in the community fear the end stage of life, not because we’re necessarily afraid of dying but because we fear such things as loss of mental faculties, control, and dignity, being a burden on family, and not receiving adequate pain relief. As competent adults, we do have a say in how and where we are to die, and whether to accept or refuse life-prolonging treatment.

Community Connection is a series of informational talks that will assist you in understanding how to take more control over the end of life wishes, through understanding what Palliative Care is, who is involved in care, staying well for as long as possible, supporting others, what is advance care planning and how to have conversations around end of life wishes.

Talks will be suitable for all of those in the community who want to understand more about planning ahead, how to support themselves and/others to get on and live life now.

Community Connection: End of Life Checklist

Topic Covered:

• Your documents – Planning and decision making & financial- i.e., ID, Will, Power of Attorney, Advance Care Directive, Enduring Guardian, financial info

• Your loved ones – i.e., Discussion of end-of-life plans, emotional will

• Your send-off – Type of funeral, burial

• What else? – i.e., passwords, pets, social media

• Resources – guides to help you plan ahead
