Palliative Care in Aged Care

Palliative care in aged care is not a specialised add-on. It is core business.

As people age, many live with multiple chronic conditions, increasing frailty, and a gradual decline in health. For a significant proportion of residents, aged care is where they will spend the final chapter of their lives. This means aged care services are not just places of support and accommodation. They are also places of care at the end of life.

Palliative care in this setting focuses on quality of life. It is about recognising when a person is approaching the end of life and responding early, not just in the final days. This includes managing pain and symptoms, supporting emotional and spiritual wellbeing, and ensuring care aligns with the person’s values, preferences and goals.

Done well, palliative care in aged care reduces unnecessary hospital transfers, prevents distress, and supports people to remain in familiar surroundings with those who know them. It also supports families and carers, helping them understand what is happening, make informed decisions, and feel confident that their loved one is receiving compassionate, appropriate care.

This is not without its challenges. Workforce capability, confidence in recognising deterioration, communication with families, and access to specialist support can all impact the quality of care delivered. Without a clear, consistent approach, opportunities to provide timely and person-centred care are missed.

The opportunity is clear. Embedding a palliative approach across aged care services strengthens care quality, supports staff, and delivers better outcomes for residents and their families. It shifts the focus from reacting to crisis, to planning, preparing and caring well.

Palliative care in aged care is about getting the fundamentals right. Early conversations. Clear plans. Skilled staff. Compassionate care.

And ultimately, ensuring that every person can live well, right to the end.
