

Advance Care Directive
Once you have taken the first step and started a conversation, you may be ready to formalise your plan and document your wishes using the Tasmanian Advance Care Directive (ACD).
In Tasmania, you can legally document your wishes using the Tasmanian Advance Care Directive. On Monday the 21st November 2022, the Guardianship and Administration Amendment (Advance Care Directives) Act 2021 commenced. This change means that an Advance Care Directive has a legal basis and provides obligations under law for health practitioners to follow an advance care directive where reasonable.

We suggest you review and update your Advance Care Directive regularly especially if you have changes in your health status, a change in your condition, a new diagnosis, your social circumstances change or if your preferences change.
It is important that you give a copy to:
- Your substitute decision-maker
- Your GP or treating doctor
- Local hospital
- Upload a copy to MyHealth record
